Complete this entry form to participate in the 4th Annual Exhibit of The National Arts Program for the Huntsville Hospital Health System employees and their family members.  There is no fee to enter the program.  


  • Participants must be an employee, contract employee or an immediate family member of an employee/contract employee of Huntsville Hospital, Madison Hospital, or Huntsville Hospital for Women & Children.
  • Only 2 entries per artist will be admitted.
  • All entries submitted must be the original work of the applicant completed within the last three years.
  • The National Arts Program and the Huntsville Hospital Health System reserve the right to disqualify entries they consider to be unacceptable or inappropriate for a public exhibit.

  • Amateur – a person with little or no experience

Artwork will be judged in the classification of the artist’s selection; however, in some cases, the exhibition judges will make the final determination.
Preparing Entries
  • Entrants may submit only two entries which fit into the listed medium categories.
  • All artwork will need a NAP identification label placed on the back (provided at drop off).
  • All 2-dimensional entries should be matted, framed and/or ready to hang with wire securely on the back. Framed entries must use plexiglass. No glass will be accepted.
  • Only 3-dimensional entries that are “ready to hang” or in a display box with wire securely on the back will be accepted.
  • All work must measure no more than 36” in any direction or weigh more than 40 pounds unless otherwise stipulated.
  • Artwork that does not meet size limitations or specifications will be disqualified before judging.

Rules of Exhibit

  • Deadline This Registration Form must be received no later than October 11, 2021 at 9:00am (EST)Space is limited. Only the first 150 artists will be accepted.
  • Delivering Entries Employees can drop off artwork at any of the following delivery options. Huntsville Hospital: Main Cafeteria/Patio on Monday, October 4 from 8am - 12pm, Tuesday, October 5 from 8am - 12pm or Wednesday, October 6 from 3pm - 7:30pm.Huntsville Hospital for Women & Children: Drop-off will be available in the Basement Classroom on Monday, October 4 from 3pm - 5pm and Tuesday, October 5 from 1pm - 5pm. Madison Hospital: Madison Hospital Board Room on Thursday, October 7 from 10am - 2pm. Please Note: Due to Covid restrictions there are limits on visitation to the hospital, for this reason we ask that all family members give their artwork to the hospital employee to bring to designated drop-off locations. You may email to arrange an option for drop-off outside of the above dates and times.
  • Reception & Awards Reception and awards presentation information is to be determined. Participants will be notified via email when details are available.
    • Prizes will be awarded by classification:
      • Adult (Amateur, Intermediate and Professional): 3 - First Place Awards of $350; 3 - Second Place Awards of $250; and 3 - Third Place Awards of $150
      • A Best of Show Award of $400
      • Youth / Teen : 2 - First Place Awards of $150; 2- Second Place Awards of $100; and 2 - Third Place Awards of $50 
      • An Art Education Award of $200
    • Honorable Mentions will be awarded at the discretion of the judges.
    • Awards are limited to one cash award per artist per classification.

  • Public Exhibit The exhibit is free and open from October 18 - December 16, 2021. The exhibit is located in the tram hallway of Huntsville Hospital. Artwork from Madison Hospital employees will then be exhibited at Madison Hospital from December 21 - January 4. Artwork will be exhibited at Huntsville Hospital for Women & Children from December 17 - January 27.
  • Retrieving Entries All Huntsville Hospital employees must pick up their artwork at Tram Hallway at Huntsville Hospital on Friday, December 17 from 7am - 2pm. Please be advised that Arts in Medicine staff will be onsite to assist with retrieval. Please do not take down your artwork without speaking to a representative of AIM. All Huntsville Hospital for Women & Children employees are responsible for retrieving their artwork from the W&C Tram Hallway on Friday, January 28 from 8am - 12pm. All Madison Hospital employees may picked up their artwork by contacting Betty Madison ( starting on January 4, 2022. Neither Huntsville Hospital Health System nor The National Arts Program will be responsible for unclaimed works.
  • Liability Every attempt will be made to preserve, secure and protect each entry submitted; but neither the National Arts Program nor Huntsville Hospital Health System can be held responsible for damages, loss of art entries, or changes/cancellation of this exhibit due to low participation or unforeseen circumstances.  The placing of art in the exhibition constitutes an agreement on the part of the entrant with conditions herein set forth.  The National Arts Program, Huntsville Hospital Health System and Huntsville Hospital Foundation reserve the right to publish for publicity purposes reproductions of the entries.  The National Arts Program in Huntsville, AL is a public exhibit and photographing/filming of the exhibit is encouraged.
