Complete this entry form to participate in the 6th Annual Exhibit of The National Arts Program® for the Childern’s Hospital of Michigan, MI employees, patients, volunteers and their immediate family members. There is no fee to enter the program.

Eligibility

Participants must be an employee, patient, volunteer or an immediate family member of an employee, patient, or volunteer of the Children’s Hospital of Michigan, MI.
All entries submitted must be the original work of the applicant completed within the last three years.
Not accepted: kits; paint-by number pieces; photocopies or photographs of artwork; magazine or book illustrations, random snapshots.
The National Arts Program® and the Children’s Hospital of Michigan reserve the right to disqualify entries they consider to be unacceptable or inappropriate for a public exhibit.

Artwork will be judged in the classification of the artist’s selection; however, in some cases, the exhibition judges will make the final determination.

Preparing Entries

Submit only entries which fit into the listed medium categories.
All artwork will need an NAP identification label placed on the back (provided at drop off).
All 2-dimensional entries should be matted, framed and/or ready to hang with wire securely on the back.
All work must measure no more than 48” in any direction or weigh more than 50 pounds unless otherwise stipulated.
Artwork that does not meet size limitations or specifications will be disqualified before judging.

Rules of Exhibit

Deadline This Registration Form must be received no later than February 20, 2012.
Delivering Entries All entries must be delivered to Grace Serra, Art Advisor at the International Guest Housing located at 3737 Beaubien on Monday, February 27, 2012 between 9am - 11am or 3pm - 5pm. Or by appointment only.
Reception & Awards A reception and presentation of awards will be held at the Children’s Hospital of Michigan Auditorium on June 25, 2012 at 4pm.
- Prizes will be awarded by classification:
  - Adult (Amateur, Intermediate and Professional): 3 - First Place Awards of $300; 3 - Second Place Awards of $200; and 3 - Third Place Awards of $100
  - Youth / Teen : 2 - First Place Awards of $75; 2- Second Place Awards of $50; and 2 - Third Place Awards of $25
  - A Best of Show Award of $300 will be presented
- Additional awards including Honorable Mentions and Scholarships will be awarded at the discretion of the judges.
Public Exhibit The exhibit will be open to the public from March 5 - June 25, 2012. The exhibit is free to the public.
Retrieving Entries Each artist is responsible for retrieving his/her artwork from the International Guest Housing on June 27, 2012 between 9am - 11am or 3pm -5pm. Or by appointment only. No work is to be removed prior to the end of the exhibition. Neither the Children’s Hospital of Michigan nor The National Arts Program® will be responsible for unclaimed works.
Liability Every attempt will be made to preserve, secure and protect each entry submitted; but neither The National Arts Program® nor the Children’s Hospital of Michigan be held responsible for damages, loss of art entries, or the cancellation of this exhibit due to unforeseen circumstances. The placing of art in the exhibition constitutes an agreement on the part of the entrant with conditions herein set forth. The National Arts Program Foundation and the Children’s Hospital of Michigan reserve the right to publish for publicity purposes reproductions of the entries. The National Arts Program® at the Children’s Hospital of Michigan, MI is a public exhibit and photographing/filming of the exhibit is encouraged.
