Complete this entry form to participate in the 11th Annual Exhibit of the National Arts Program for Ann & Robert H. Lurie Children's Hospital of Chicago employees, physicians and their family members. There is no fee to enter the program.

Eligibility

  • Participants must be an employee, physician or an immediate family member of an employee or physician of Lurie Children’s.
  • A maximum of 2 entries (artworks) per artist will be admitted.
  • Space is limited. Only the first 100 artists will be accepted.
  • All entries submitted must be the original work of the applicant completed within the last three years and not previously submitted.
  • The National Arts Program and Lurie Children’s reserve the right to disqualify entries they consider to be unacceptable or inappropriate for a public exhibit.

Entries
  • Entrants may submit a maximum of two entries which fit into the listed medium categories.
  • All artwork will need an NAP identification label placed on the back (provided at drop off).
  • All 2-dimensional entries should be matted, framed and ready to hang with wire securely on the back.
  • Only 3-dimensional entries that are ready to hang and measure no more than 8” in depth will be accepted.
  • Maximum length for entries is 36”; maximum depth is 8”; maximum weight is 20lbs.
  • Artwork that does not meet size limitations or specifications will be disqualified before judging.

Rules of Exhibit
  • Deadline This Registration Form must be received no later than May 13, 2024. Space is limited. Only the first 100 artists will be accepted.
  • Delivering Entries All entries must be delivered to the 11th floor Ryan Family Lobby (In Sky Garden) of Lurie Children's during the following times:

         Monday, June 24 between 10:30am-12:30pm

         Tuesday, June 25 between 9:00am-11:00am

         Thursday, June 27 between 11:00am-1:00pm

         All participants will be required to sign an Ann & Robert H. Lurie Children’s Hospital of Chicago release form at drop off.
  • Awards Presentation & Virtual Exhibit Tour A presentation of awards will be held in the 11th Floor Conference Center at Lurie Children’s on Monday, July 22nd beginning at 4pm.
    • Prizes will be awarded by classification:
      • Adult (Amateur, Intermediate and Professional): 3 - First Place Awards of $350; 3 - Second Place Awards of $250; and 3 - Third Place Awards of $150
      • A Best of Show Award of $400
      • Youth / Teen : 2 - First Place Awards of $150; 2- Second Place Awards of $100; and 2 - Third Place Awards of $50
      •  An Art Education Award of $200
    • Honorable Mentions  Ribbons will be awarded at the discretion of the judges.
    • Awards are limited to one cash award per artist per classification.
  • Exhibition The exhibit will be on display from July 22, 2024 through May 31, 2025 along the 11th floor staff corridors. 
  • Retrieving Entries Each artist is responsible for retrieving their artwork from Lurie Children’s during the last week of May 2025. Dates and times are to be determined and all participants will be notified.  No work is to be removed prior to the end of the exhibition. Neither Lurie Children’s nor the National Arts Program will be responsible for unclaimed works. 
  • Liability Every attempt will be made to preserve, secure and protect each entry submitted; but neither the National Arts Program nor Lurie Children’s can be held responsible for damages, loss of art entries, or changes/cancellation of this exhibit due to low participation or unforeseen circumstances.  The placing of art in the exhibition constitutes an agreement on the part of the entrant with conditions herein set forth.  The National Arts Program and Lurie Children’s reserve the right to publish for publicity purposes reproductions of the entries.  The National Arts Program at Ann & Robert H. Lurie Children’s Hospital of Chicago, is a public exhibit and photographing/filming of the exhibit is encouraged.
